In the event that we do not participate with your insurance plan or if you do not have insurance coverage, you will be considered a "Self-Pay" patient. You will need to file your own insurance. Payment is due in full at the time service is rendered.

- Medicare Breezewood Family Healthcare is not accepting new Medicare patients at this time. This information is to assist our current Medicare patients. If you have medicare and desire to be seen at Breezewood Family Healthcare, please visit this site often, we will advise if we are accepting additional medicare patients.

Breezewood is happy to announce we are resuming care for TRICARE Prime patients and we are continuing to accept Gyn TRICARE Prime paitents.
You will need to change your PCM before we can schedule you for an appointment. You can do this in one of three ways:
1. Contact TRICARE at 1-800-874-2273, or
2. Go online to www.healthnetfederalservices.com, or
3. You may go to the TRICARE offices on Ft. Bragg.
