Below is a list of insurance companies we currently participate with.  We will file the insurance companies listed below for you and you will be responsible for paying any deductible or co-payment amounts.

In the event that we do not participate with your insurance plan or if you do not have insurance coverage, you will be considered a "Self-Pay" patient.  You will need to file your own insurance.  Payment is due in full at the time service is rendered.
