Prior to every visit please download and fill out BFH_Medication_Chart, this will assist your practitioner in assuring you the best care. If you are scheduled for your annual physical please fill out the BFH Medical History Forms and the BFH_Medication_Chart and bring them to your visit. Having these forms completed prior to your visit could reduce your wait time.
