Breezewood Healthcare has CLOSED
After 15 years of serving our community, Breezewood Healthcare has closed. We would like to thank all of our patients for the privilege and the trust you put in us to care for your health needs.
Obtaining your Medical Records.
Breezewood does not have a phone service to take your requests, however we can be contacted via two methods:

- Fax: 910-401-1284
We will be processing medical records requests through out the remainder thru March of 2019. Please submit you medical records request throught the 2 methods stated above. You can obtain a copy by clicking on "Patient Forms" tab on the left side of the page and then selecting "Medical Release Form." Please ensure that you have your personal email address on your form so that we can get your records to you.
We will send you an e-mail confirmation that you records have been processed.
We will no longer be sending records to your new providers, however, we will process your records and send you an encrypted copy. This will eliminated may delays as we will not be restricted by the size and time we fax copies of medical records.
As we move forward we will be sending only electronic copies via an encrypted email. We will also be sending and encrypting the last two years, from your last visit, of your medical records.
If you need some of your records immediately you can access your patient portal and obtain a visit summary and labs. You can also obtain your labs through labcorp's portal at https://patient.labcorp.com.
We are currently working diligently to get everyone's records to their new providers but are running behind. If you are requesting a personal copy instead, we are even further behind as this requires many more steps. If it has been in excess of 45 days please e-mail office-manager@breezewoodhealthcare and we will see if it is just a delay, missing information, missed your request or do not have your request.
